As a Physical Therapist you will:

Review physician’s referral and patient’s medical records to help determine diagnosis and physical therapy treatment required. Evaluate, care for and direct treatment for patients.

Plan, prepare and perform individually designed treatment programs to maintain, improve and/or restore physical functioning, alleviate pain and/or prevent physical dysfunction in patients.

Identify and document goals, anticipated progress and plans for re-evaluation.

Evaluate effectiveness of treatment at various stages and adjusts treatment to achieve maximum benefit.

Provide patients with a meaningful rehab experience utilizing excellent clinical skills and customer service.

Qualifications:

Bachelor’s degree (B.A.) from a college or university accredited by the Commission on Accreditation 

Position requires a minimum of two years job-related experience or equivalent combination of education and work experience. Prior clinical experience a must.

Strong understanding of state, federal and local regulations pertaining to homecare & outpatient rehabilitation.

Understanding of clinical documentation requirements utilizing electronic health records for Medicare and other payors in accordance with guidelines established by Medicare, Medicaid, other payors and the APTA.

Strong communication skills for accurate, concise, and organized verbal and written interactions with patients and co-workers.
